What’s in the pack

  • An appointment checklist and space for your main concerns.
  • A two-week symptom and sleep diary.
  • Questions to ask and space for your agreed plan.

Use as much or as little as you like. You don’t need to complete the diary before booking. Keep your completed copy private.

A few common questions

Do I need to do the assessment before booking?

No. You can book without completing the assessment or the pack. They are optional ways to organise your thoughts.

Can I bring this pack to my usual GP?

Yes. Print it or keep it on your device and bring it to whichever GP you see.

What if I’m worried about a symptom?

Book a GP appointment rather than waiting to finish a diary. Any bleeding after menopause needs medical review. Read Healthdirect’s advice about bleeding after menopause. For a medical emergency, call 000.
