Our detailed health assessments are tailored to your age and stage of life. This service is offered with no out-of-pocket expense to you, and involves spending between 30-60 minutes with our practice nurse, followed by a consultation with your doctor.

What is involved in a health assessment?
What should I expect from my health assessment?
Health assessments provide a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve your health and quality of life.
The purpose of this health assessment is to help identify any risk factors you may have that may require further health management. In addition to assessing your health status, a health assessment is used to identify a broad range of factors that may influence your physical, psychological and social functioning.
Your practice nurse will spend up to an hour with you, ask a series of questions to better understand your health needs, and administer any appropriate screening tools. You will then see your GP, who will help you to develop a personalised management plan.
Your check will depend on your age and circumstances. Your practice nurse and GP might:
- check your blood pressure
- check blood sugar levels
- measure your height and weight
- do a blood test
- do a urine test
- ask about the health of your family
- talk to you about your health priorities and goals
- do other tests, as needed.
Remember that it’s your check, so please tell us about any worries you have.

Health Assessments Offered:
45-49 Year Old Health Assessment
A one-off health assessment available to people between 45-49 years of age. It is designed to identify health risk factors, and assist in the prevention or early diagnosis of chronic conditions, such as diabetes.
Over 75 Health Assessment
A comprehensive annual at-home or in-practice health assessment designed to help older Australians stay independent, safe and confident about their health.
Aboriginal and Torres Strait Islander Health Assessment
Available annually to all people of Aboriginal and Torres Strait Islander descent, of all ages.
Comprehensive Health Assessment Program (CHAP)
Designed for children and adults with a disability. The assessment may include an assessment of your teeth, hearing, eye sight, nutrition, any
swallowing problems, immunisations and so on. Your nurse and GP will advise on preventative health care, treatment and any other medical tests that are needed.
Booking your Appointment
How do I book an appointment?
The best way to book an appointment is by calling our friendly reception team on 02 9680 9644. Our receptionists will work with you to identify a suitable time for your health assessment.
Remember, you will be booked in for approximately 30-60 minutes with your practice nurse, and 15 minutes with your GP. To save time waiting on the day of your consultation, we suggest you call us to ensure that your nurse and doctor are running on time.
Our practice nurse offers at-home health assessments for people over the age of 75. The added benefit of this service is the opportunity to review your medications and assess your falls risk and safety at home.
For other age groups, we prefer to see you face-to-face in the practice to ensure that we are able to accurately capture your health needs.
If you feel pressured for time, we may be able to accommodate your needs in other ways. For example, you may wish to book an initial consultation with your practice nurse, and follow up with your GP on another date.
If you have any other needs, please let reception know, and we will try our best to accommodate your request.
