How to Book your COVID-19 Vaccine

How to Book

We are now accepting bookings for individuals listed in phase 1B. Our approved process for vaccinations at Rosedale Medical Practice ensures your safety is our first priority. To book your appointment:

 

1. Book your appointment online by clicking on the ‘Book Now’ button below.

You will be asked to book for both your first and second dose. Your second dose will be approximately 12 weeks after your first.

2. Complete the following forms (you will receive these forms via email, or can click on the links on the left):

– The New Patient Registration Form

– COVID-19 Vaccine Consent Form

You will need to complete both of these forms for your booking to be successful.

3. Arrive 10 minutes early for your appointment

Our reception team will check you in and assist you on arrival. COVID-19 vaccine appointments generally run on time and our community has found the process to be efficient, safe and comfortable. You will be provided with a certificate confirming that you have received the vaccination.

We suggest you visit the Australian Government Department of Health website for the most up-to-date information about the COVID-19 vaccinations.

SmartVax

We have implemented the AusVaxSafety vaccine monitoring and surveillance program (SmartVax) in our practice. After your vaccine, you will receive a text (SMS) and questionnaire from SMARTVAX on days 3, 8 and 42. Please respond to this so that you can tell us if you have had any side effects like a sore arm, headache, fever, fatigue, chills, muscle pains, joint pains or something else.

For anything that you are worried about, please book an appointment with your regular GP. If it is an emergency, call 000 or attend your local hospital emergency department.

April 2021 Update

The Australian government has made some changes to the way the COVID-19 vaccine will be rolled out. Here’s what we understand so far:

What are the changes?

1. If you’re under 50 years of age and have not yet received the Astra Zeneca vaccine, you will most likely receive the Pfizer vaccine instead.

2. If you’re under 50 years of age and have received the first dose of the Astra Zeneca vaccine, you can still receive your second dose of the Astra Zeneca vaccine.

3. If you’re over 50 years of age, the Astra Zeneca vaccine remains the recommended option.

Why?

There have been a small number of cases of blood clots around the world. They are rare but serious. They seem to be more common in younger people. They could possibly be linked to the Astra Zeneca vaccine, but we’re not yet sure. The Australian government is taking the above actions to be safe.

What do I need to do?

At this stage, if you are under 50 years of age we suggest you discuss your risks with your GP before you consider having your COVID-19 vaccine. This is early advice and we will keep you informed once we know more.
